MEETING LOCATION
MEETING LOCATION & MEETING SESSION TIME FOR VIRTUAL SESSIONS
The following conditions are required to ensure the safest virtual therapeutic environment possible. The client must:
Not adhering to any of the above meeting location will result in late cancellation of the session and the full agreed upon session rate will be charged.
INSURANCE
University Interns are not in network with any insurance carriers. Please read over the Licensed counselor's page to learn more about which insurance plans they are in network with.
APPOINTMENTS AND CANCELLATIONS
Cancellations will be subject to a charge of the agreed upon session rate for all clients if Received after the 24-hour window (this include the full insurance rate) or if you no show. You can cancel directly through the portal by canceling the appointment prior to 24 hours before your scheduled session. Once you're in the 24-hour window, You must send a message through the portal or call and speak with me directly. Voicemails will not be accepted. Please DO NOT send a text message to the number provided. The line does not have text features.
You will receive an email reminder, voice call or text reminder (which ever you sign up for, but you must choose one) 48-hours & 15 minutes prior to your scheduled appointment. All reminders sent through the portal are timestamped.
FMLA &/OR OTHER PAPERWORK
At this time, only therapists who are fully licensed are able to complete any FMLA, provide any type of supporting letters or disability paperwork. If you are unsure about whether or not a clinician is fully licensed, revisit your therapist's details. If it says that they have a supervisor, then they will not be able to complete any paperwork. If your therapists is fully licensed, please keep reading.
If you request any letters, forms, or any other paperwork to be completed, such as, but not limited to FMLA or disability forms, please be advised that there is a fee for all paperwork. Our fee is $150.00 per hour for all clients. The fees will be paid by you the client. FMLA paperwork generally requires a minimum of 30-60 minutes to complete, due to the need for supporting clinical documentation. Disability paperwork often takes longer to complete and may require additional assessments beyond my regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to HR or other non-clinical calls) not directly related to scheduling or cancellations, will also be assessed based on a rate of $150.00. Only licensed clinicians can complete FMLA or disability paperwork. Students will not be allowed to complete any form of paperwork of this nature.
We will not complete any FMLA, disability, other paperwork or letters of support unless we have met with you for at least 2 sessions. We will also not complete any FMLA or disability paperwork if we do not believe we can support it based on what you have presented at intake and during sessions. If your FMLA or short-term disability is approved, weekly sessions will be recommended in order to address your current needs. Before completing any paperwork,we will let you know the amount of time we will need to complete the document so that payment can be made. Documents will be completed and submitted within 5 business days (Monday-Thursday).
LITIGATION POLICY AND FEES FOR COURT-RELATED SERVICES:
Fees for litigation related services will also be outline in the practice policy that you are required to sign.
