SESSION FEE Sessions are $120 per 60 minute session (unless another fee was agreed upon at intake). There are a limited amount of sliding scale slots available. Payments are taken the day of your scheduled session.
The standard meeting location for therapy is in a private location to help ensure that confidentiality can be upheld. Therapy sessions will not be conducted while you are out in public places. If you are driving, we must have a telephone session to ensure your safety while driving. Excessive telephone sessions may result in terminating services until you are able to commit to the agreed upon session time.
Blue Cross Blue Shield & United Health Care is currently accepted at Wanda Hudson Consulting. You are financially responsible for copayments, coinsurance or deductibles. Insurance clients are required to have a credit card/debit card on file for cancellation fees, copayments, coinsurance or deductibles. I will call to verify benefits prior to your first session. There are times however, that I may file a claim and find out that the amount owed may be different from the original amount shared. Your employer has agreed to the plan and benefits. Every service allowed is not disclosed when a provider does an initial benefits and eligibility check. By signing this, you are also agreeing to paying any additional fees in the event that a claim is denied due to diagnosis exclusions, misinterpretation of benefits by the provider or insurance carrier.
APPOINTMENTS AND CANCELLATIONS
Please remember to cancel or reschedule 24 hours in advance (Prior to your 24 hour reminder). You will be responsible for the fee if cancellation is less than 24 hours.
Excessive cancellation of biweekly sessions may result in early termination or referral resources until you are able to commit to the agreed upon session frequency. It is important to be ready to commit to the agreed upon session frequency per your treatment plan. There are fees for late cancellations, no shows and showing up late or ending the session early for clients who use insurance. These fees will be outlined in the practice policy document that you sign.
FMLA &/OR OTHER PAPERWORK
If you request any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms, please be advised that there is a fee for paperwork. My fee is $120.00 per hour for self pay, $87.67 per hour for BCBS, $109.59 per hour for United Health & $90 per hour for Lyra. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete, and may require additional assessments beyond my regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls HR or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $120.00, with a minimum fee of $25.00.
I will not complete any FMLA, disability, other paperwork or letters of support unless I have met with you for at least 4-6 sessions. I will also not complete any FMLA or disability paperwork if I do not believe I can support it based on what you have presented at intake and during sessions.
LITIGATION POLICY AND FEES FOR COURT-RELATED SERVICES:
Fees for litigation related services will also be outline in the practice policy that you are required to sign.
